COVID-19 Information

Dear Resident and/or Resident Representative,

As per the direction of the Department of Health, we have developed a plan to cautiously begin to reopen our skilled nursing facility through a step by step process as recommended by the Department of Health. Our Implementation Plan is enclosed for your review and will also be available and updated on our website https://themeadowsdallas.com for your reference. Please note any updates to our plan will be reflected on our website. If there are any confirmed cases of the virus among residents or employees, this information will be posted on our website. Please note our Implementation plan is subject to change based on CDC Guidelines & Orders of the Secretary of the Department of Health and Governor Wolf.

It is important for you to read the Implementation Plan in its’ entirety so that you understand the process. Visitors are expected to adhere to all the visitation requirements discussed in the plan. These measures are in place to protect your loved ones from this virus. A detailed explanation of each Step is enclosed for your information as per Department of Health Guidelines.

Remember to continue to do your part to keep Pennsylvanians safe and take the following steps to avoid spreading COVID-19: Maintain social distance, approximately 6 feet or 2 meters, from others, wear a cloth face covering when in public and wash your hands often, using alcohol-based hand sanitizer or soap and water for twenty seconds. Our tentative date for allowing visitors under STEP 2 of the Pennsylvania Department of Health Guidance is expected to be August 3, 2020. Please check website for any updates.

Steps to Reopen

The following Steps provide an incremental lifting of restrictions.


Dining1

Communal dining is limited to residents unexposed to COVID-19. Those residents may eat in the same room with social distancing (limited number of people at tables and spaced by at least six feet). Adhere to the Precautions When MealsAre Served in a Common Area in Section 5 of this guidance.
All other residents must adhere to the restrictions in Section 5, Dining Services When Not In the Reopening Process.


Activities

Step 1
Limited activities may be conducted with five or less residents unexposed to COVID-19. Social distancing, hand hygiene, and universal masking are required.

Step 2
Limited activities may be conducted with ten or less residents unexposed to COVID-19. Social distancing, hand hygiene,and universal masking are required.

Step 3
Activities may be conducted with residents unexposed to COVID-19. Social distancing, hand hygiene, and universal masking are required.


Non-Essential Personnel

Step 1
Adhere to restrictions in Section 4, Visitor Policies When not In the Reopening Process.

Step 2
Non-essential personnel are allowed as determined necessary by the facility, with screening and additional precautions including social distancing, hand hygiene, and universal masking. Services for exposed residents adhere to the restrictions in Section 4, Visitor Policies When Not In the Reopening Process.

Step 3
Non-essential personnel are allowed with screening and additional precautions including social distancing, hand hygiene, and universal masking. Services for exposed residents adhere to the restrictions in Section 4, Visitor Policies When Not In the Reopening Process.


Volunteers

Step 1
Adhere to restrictions in Section 4, Visitor Policies When not In the Reopening. Process.

Step 2

Volunteers are allowed only for the purpose of assisting with visitation protocolsand may only conduct volunteer duties with residents unexposed to COVID-19. Screening, social distancing, and additional precautions including hand hygiene and universal masking are required.

Step 3
Volunteers are allowed but may only conduct volunteer activities with residents unexposed to COVID-19. Screening, social distancing, and additional precautions including hand hygiene and universal masking are required.


Visitors

Step 1
Adhere to restrictions in Section 4, Visitor Policies When not In the Reopening Process.

Step 2
Outdoor visitation (weather permitting) is allowed in neutral zones to be designated by the facility. If weather does not permit outdoor visitation, indoor visitation is allowed in neutral zones to be designated by the facility and defined in their Implementation Plan. Visitation is limited to residents unexposed to COVID-19.
Visitation for exposed residents adhere to the restrictions in Section 4, Visitor Policies Not Impacted by the Reopening Process.

Step 3
Indoor visitation is allowed in neutral zones to be designated by the facility. Visitation is limited to residents unexposed to COVID-19. Visiting in a resident’s room (within facility’s established protocols) is permitted only if the resident is unable to be transported to designated area. Cross-over visitation is only permitted if there is no new facility onset of COIVD-19 in the facility in which the cross-over visitor resides. Visitation for exposed residents adhere to the restrictions in Section 4, Visitor Policies Not Impacted by the Reopening Process.


Outings

Step 1
Adhere to restrictions in Section 4, Visitor Policies Not Impacted by the Reopening.

Step 2
Adhere to restrictions in Section 4, Visitor Policies Not Impacted by the Reopening.

Step 3
Outings are allowed only for residents unexposed to COVID-19. Outings limited to no more than the number of people where social distancing between residents can be maintained. Appropriate hand hygiene, and universal masking are required.
Outings for all exposed residents adhere to the restrictions in Section 4, Visitor Policies Not Impacted by the Reopening Process.
